I received one of those a couple of years back. I tossed it aside. They kept sending me letters, pretty much telling me to fill it out or else.

So basically they want you to spend 1 hour filling out information for free, just to sell all your information to marketing agency (which you'll never see money from), all so that you can be bombarded with targeted ads.
